▪ NO SMOKING/VAPING within the hospital premises.
Our hospital is equipped with fire alarms and water sprinklers that may trigger its activation anytime. A corresponding fee will be charged to all violators.
▪ NO BRINGING IN AND DRINKING OF ALCOHOLIC BEVERAGES in the hospital.
▪ NO SHARPS AND DEADLY WEAPONS/ FIREARMS/ EXPLOSIVES allowed within the hospital.

▪ Hospital Canteen (located on the second floor beside Central Supplies-CSR) also provide rice meals and merienda for your needs.

Monday to Friday: 8AM to 5PM
Weekends (Saturday & Sunday): 7AM to 4PM
▪ Vendo service machine now available at the hospital lobby (upper ground floor) providing cold juices, soft drinks, chips and biscuits too. Service is available 24 hours.
▪ FOOD DELIVERIES/ITEMS BROUGHT BY RELATIVES will only be accepted at the lobby guard during the following times:
Breakfast: 7AM to 8AM
Lunch: 10AM to 12 NN
Dinner: 5PM to 7PM
▪ No food courier service is allowed within the hospital premises.
▪ Food and items will be collected in the guard of the lobby area.

▪ The hospital shall not be held liable for any loss or damage to personal belongings or valuables, including cash, credit cards, laptops, and cell phones.
▪ If a procedure (e.g., X-ray, ultrasound, operation) requires the occupant to leave the room, please lock the room and inform the nurse on duty.

To check patient eligibility for PhilHealth benefits, please present the following:
❖ PhilHealth Number/Identification Card (ID) or Member Data Record (MDR) ❖ PhilHealth Proof of Dependency:
For children below 20 years old: Birth Certificate
For spouse: Marriage Certificate

▪ Automated Teller Machines (ATMs) from Security Bank are available at the hospital's main entrance and lobby (upper ground floor) for your convenience should you require cash withdrawals within the hospital premises.

▪ For patient's safety, all equipment/devices required and or to be used by the patient should come from the hospital (e.g. nebulizer, suction machine, respirator, oxygen generator, air purifier/humidifier etc.)

▪ No cooking/ heating devices allowed (rice cooker, induction cooker, portable gas burner, electric kettle).
▪ Bringing laptops are allowed but should be declared upon entry in the hospital premises from the security guards and registered in their logbook. Should there be more gadgets found, electricity charges shall be imposed per gadget.

▪ For patient's safety, all drugs/ medicine required by and to be administered to the patient should be purchased and provided by the hospital pharmacy and the undersigned further expressly waives any option to purchase and to bring in drugs /medicine bought outside.
▪ 15% of the acquisition price of the brought-in medicine will be charged as administrative fee.

▪ Our hospital is a strong advocate of breastfeeding for newborns, and we prohibit the practice of bottle milk formula feeding and use of pacifiers in our hospital.
We have a Breastfeeding and Lactation Room on 3rd floor near the Outpatient Department for your convenience.

Please segregate your waste properly:
Black – Dry waste (paper, plastic, cans) Green – Wet waste (food scraps, peelings, flowers) Yellow – Infectious waste (used dressings, plasters/cotton balls/tissue/napkin/gloves/mask)

▪ In case of an emergency (e.g., fire, earthquake), safety measures such as automatic water sprinklers, fire extinguishers, fire alarms, and emergency exit doors are available in our facility.
▪ Stay calm, do not panic, and follow instructions from our safety officer. Our staff will assist you during any emergency.

▪ Diagnostics HMO Approval is located on the upper ground floor adjacent to the admitting section.
▪ Consultation/Check-Up HMO Approval is on 4th floor HMO office.
▪ Our HMO Department can assist patients regarding the validity/accreditation of one's card, coverage of illness, securing letter of authorization (LOA). Kindly prepare your HMO card, Government issued ID and Doctor’s request for approval.

▪ Our Medical Records Department is located on the 4th floor.

Saturday- 8:00 AM to 4:00 PM
▪ Inpatient request of confinement records (laboratory results, ultrasound and x-ray results), clinical abstracts, medical certificate, birth and death certificate, should be coursed through the nurse-on-duty at least a day prior to your discharge.
▪ Requests made after discharge would be directly sent to the Medical Records section.
▪ If the request is made through the patient's representative, please provide an Authorization Letter from the patient together with the valid id of the patient and the authorized representative.
▪ Release of requested medical records would be 3-4 days.
For Birth Certificate: The Certified True Copy may be claimed at the medical records department once processed. The Original Parent's Copy can be claimed at the Registrar's Office in Cainta Municipal Hall, 21 days after birth or on or before 30 days.
